Medical Social Workers

The Medical Social Worker is a qualified professional person who provides social case work and prepares social histories for patients and family receiving Nursing or other services from the agency and who have problems in social functioning as it relates to their health and well-being.

These workers are the link between the patient and his/her family and the community resources available including meal delivery, counseling and patient advocacy.

Job Description:

  1. Graduate with a Master’s Degree from an accredited school of social work.
  2. One (1) year of social work experience in a health care setting.
  3. Registration to practice as a Medical Social Worker in the state of practice, if the state has registration practices.

Reports To: The Director of Nursing and designated Nursing Supervisor.
