Speech Therapist/Pathologist

1. Complies with ethical and professional standards of conduct as set forth by the American Speech Hearing and Language Association.

2. Evaluates patient speech and language abilities, both defects and assets.

3. Plans and provides rehabilitative services for speech, language and dysphagia disorders.

4. Participates in the development of the Physician’s Plan of Care.

5. Develops, documents, implements and re-evaluates the Patient’s Plan of Care.

6. Records clinical/progress notes to the patient’s clinical record weekly.

7. Records type of treatment and patient’s reaction to it. Reports progress and/or regression.

8. Maintains adequate records on all patients.

9. Utilizes community resources and agency personnel by proper referral.

10. Selects and administers diagnostic and therapeutic techniques and materials.

11. Instructs and counsels other health team personnel and family members in methods of assisting patient in improving, correcting and accepting his/her disabilities.

12. Plans for patient’s discharge, prepares discharge summaries and instructions.

13. Attends agency service meetings and participates in staff development activities.

14. Performs related and other duties as required.
