Occupational Therapist

1. Complies with ethical and professional standards of conduct as set forth by the American Association of Occupational Therapists.

2. Evaluates and establishes goals based on the patient’s current level of functioning and potential for improvement.

3. Assists in the development of the Physician’s Plan of Care.

4. Develops, documents, implements and re-evaluates the patient’s Plan of Care.

5. Guides and instructs the patient and prescribes therapeutic self-care and creative activities that are directed towards improving independence and physical and mental functioning.

6. Establishes household management routines.

7. Reports to the Physician the patient’s reactions to the therapy program or any changes in the patient’s condition.

8. Records clinical/progress notes in the patient’s clinical record weekly.

9. Instruct patients and their families in the use of prosthetic and orthotic or assistive devices such as canes, walkers, wheelchairs and so forth.

10. Instructs the family in the patient’s total occupational therapy program.

11. Instructs other health team personnel in the care of patients regarding occupational therapy.

12. Plans for patient’s discharge, prepares discharge summaries and instructions.

13. Attends Case Management/Coordination meetings and other agency in-service meetings.

14. Participates in staff development activities.

15. Performs related and other development activities.
