Physical Therapist

1. Complies with ethical and professional standards of conduct as set forth by the American Physical Therapy Association.

2. Assists the Physician in evaluating client/patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests and participates in the Physician’s Plan of Care.

3. Develops, documents, implements and re-evaluates a specific Plan of Care noting all treatment modalities and frequencies.

4. Treats client/patients to relieve pain and/or develop or restore function to maintain maximum performance.

5. Make arrangements for out-patient services which cannot be given in the home.

6. Reports to the Physician the client/patient’s reaction to treatment and/or any changes in the client/patient’s condition.

7. Records clinical/progress notes in the patient’s clinical record weekly.

8. Instructs client/patients and their families in the use of prosthetic, orthotic and assistive devices (cane, walkers, wheelchairs, etc.).

9. Instructs the family in the client/patient’s total physical therapy program and evaluates the home environment.

10. Instructs other involved agency personnel in the care of client/patients receiving physical therapy.

11. Attends appropriate case conferences and In-service presentations.

12. Plans for patient’s discharge, prepares discharge summaries and instructions.

13. Participates in staff and agency development activities.

14. Performs related and other duties as required.

15. Reports to the Director of Nursing and designated Nursing Supervisor or to the Rehabilitation Manager.
